Candidate Forms

Candidate Profile

This is the Part 1 form, and the primary application document. In the Candidate Profile, applicants complete basic information such as contact details, academic information, extracurricular activities, and coursework, among other important pieces of information.

Graded Writing Sample

This is a Part 2 form, and a shared supplemental form. In the Graded Writing Sample, applicants submit an assignment, written in English, from this academic year (preferably from English class).

Recommender Forms

Previous Grades

Send this request to the school official responsible for grade reports at your current school. The school will be responsible for sending your grades from the past two years:
• If you are new to your school this year, send this request to the school official responsible for grade reports at your previous school.

Current Academic Year Grades

Send this request to the school official responsible for grade reports at your current school after the conclusion of your school’s first grading period.

Optional Additional Grades

Only share additional required grade reports here, if applicable. Send this request for additional grades to a school official only if:
• You have attended more than two schools in the past three years and need additional space to share required grade reports.
• A member school has reached out requesting a missing required grade report.

Principal/Counselor Recommendation

The Principal/Counselor Recommendation should be completed by the applicant's current principal or counselor. The principal or counselor will be asked to rate the applicant on certain academic and personality traits and optionally provide a written evaluation.

Current English Teacher Recommendation

The Current English Teacher Recommendation should be completed by the applicant's current English teacher. The teacher will be asked to rate the applicant on certain academic and personality traits and optionally provide a written evaluation.

Current Mathematics Teacher Recommendation

The Current Mathematics Teacher Recommendation should be completed by the applicant's current Mathematics teacher. The teacher will be asked to rate the applicant on certain academic and personality traits and optionally provide a written evaluation.

Personal Recommendation

The Personal Recommendation should be completed by someone that knows the applicant personally. The recommender will be asked to provide a written evaluation of the applicant's personal qualities.

Special Interest Recommendation

The Special Interest Recommendation should be completed by someone that knows the applicant in an extracurricular capacity. Special Interest Recommendations generally relate to a specific extracurricular activity that the candidate is interested in. The recommender will be asked to provide a written evaluation of the applicant in this regard.

Pre-K to 4th Parent Questions

This is a Part 2 form, and a shared supplemental form. In the Parent Questions, parents respond to a few questions related to the family and the student.
